Children and Young Adult Permission Slip
Access Authorization
The Madison Valley Public Library does not censor
what computer users choose
to view on the Internet. Some sites on the Internet are offensive to some
people.
Therefore, we require a parent or legal guardian to give written permission for
anyone under the age of 18 to use the computer with access to the Internet
available at our site. As a parent or legal guardian you are welcome and
encouraged to view the Internet with your child. To use the computer by
himself/herself at Madison Valley Public Library, this permission slip is
required.
I am the parent or legal guardian of:

We both understand that the use of the Internet
is a privilege and inappropriate use as explained in Section C of the AUP will
result in cancellation of that privilege. Everyone is expected to abide by
legal and generally accepted rules of network etiquette. We understand
that children should not give out personal information when using e-mail, chat
rooms and other forms of direct electronic communication (including instant
messaging). We have read the posted Acceptable Use Policy
and understand it. I now give my permission for him/her to use the
computer with access to the Internet by himself/herself.
